Summary

Drivers needed to work flexible shifts, providing care and support individuals coming out of hospital. You will provide person-centred care, which promotes independence through enabling approaches and connecting people to local groups and services.

Successful candidates will work as part of an integral team with the goal to enable our customers to continue to live at home.

Due to the nature of the work and the fast paced, rapid response required we are looking for people who can be flexible. You will work on set days with a willingness to pick up work at short notice.

Vacancies: Full and Part Time
Contracts: Guaranteed 16hrs per week
Shifts: Set working days, required to be available 7:00am-10:00pm
Paid holidays and bank holidays
Under 25s - your hourly rate will be no different from the rate stated below

Additions

Mileage paid at 30p per mile
20p visit payment made for every call you attend

Eligibility Required

Driving licence with access to own vehicle for work purposes

Experience Required

Experience is desirable but not essential. Full training and support will be provided if you're new to care.

The Role

The purpose of the role is to:

• Provide care and support to clients living at home and to enable them to remain as independent as
possible. The Home from Hospital Care Team will provide a service to all adults, including those with
dementia and to holistically support those clients who are unable to recover from ill health.

• Provide first response support for adults and older people on discharge from hospital or following a
period of ill health living at home, to promote independence and safeguard individuals’ well-being until a
permanent team of staff are allocated, the anticipated maximum length of time clients will be cared for
by the Home from Hospital Team is a period of 6 weeks.

•Assist the patients/clients throughout the reablement process and to care for those who are unable to
recover from ill health.

• Assist the patients/clients in achieving their agreed goals.

• Provide services within a culture of maximising independence; help individuals to maximise their levels
of independence with dignity and choice; work at all times from the assumption that ‘doing with’ is
better than ‘doing for’, help them to achieve their aim of remaining in their own home and local
community and reduce the need for long-term dependence on others.

If you are studying towards a qualification in Social Care, Nursing or Health Care this role could provide you with paid-for practical experience. We can provide you with community-based insight to align with your studies.

  • Induction includes a minimum of; 3 x half day classroom based sessions, 14 online learning units and shadowing care calls. As an employee-owned organisation, we pay everyone for successfully completing their induction.
  • We will ask you about your availability at interview to discuss a potential working pattern that meets the needs of the service and the people we support.

    Typically, we ask people to work a mixture of evenings and weekends, for example; alternate weekends, and 2 evenings per week. If you have applied for a weekend only job, then we will expect that you will be available to work weekends.
  • The majority of our colleagues tend to work on early and late shifts, some colleagues work full days.

    EARLY shifts tend to be from 7:00am to 2:00pm and LATE shifts tend to be from 3:00pm to 10:00pm
